Fetal sex from cfDNA is highly accurate from 10 weeks. The interesting part is the small number of situations where it is not, and why.

How it is determined
The test looks for Y chromosome sequences. If they are present in sufficient quantity, the result is reported as male; if absent, female. It is a presence-or-absence judgement rather than a measurement, which is why it is more robust than trisomy detection but also why the failure modes are distinctive.
Specifically, the errors run in a predictable direction. If fetal fraction is very low, Y sequences from a male fetus may fall below the detection threshold and be missed, producing a female result for a male pregnancy. The reverse — detecting Y material where there is no male fetus — requires an external source of Y DNA, which is why the causes are so specific.

That last row is worth understanding rather than treating as an error. Where cfDNA and ultrasound disagree, the appropriate response is clinical assessment, not repeated testing.

When fetal sex genuinely matters clinically
X-linked conditions. Where a family carries a condition such as haemophilia or Duchenne muscular dystrophy, knowing fetal sex early determines whether further diagnostic testing is needed at all. A female result may remove the need for an invasive procedure.
Congenital adrenal hyperplasia. In families at risk, early knowledge of fetal sex informs decisions about prenatal management, which is time-sensitive.
Interpreting other results. Fetal sex is needed to interpret sex chromosome findings sensibly.
