The most important thing to know in the first hour after a high-risk result is that it is not a diagnosis, and that a meaningful proportion of these results turn out to be wrong.

Why it is not a diagnosis

NIPT analyses placental DNA. Usually the placenta and the baby are genetically identical, but not always, and the difference is the origin of most false positives.

The main mechanisms:

  • Confined placental mosaicism — the abnormality is present in placental cells only. This is the commonest cause of a false positive.
  • Vanishing twin — DNA from a twin that stopped developing early can persist for weeks.
  • Maternal chromosomal variation — an undiagnosed variation in the mother contributes DNA that is misread.
  • Maternal malignancy — rare, but a recognised cause of unusual multi-chromosome patterns, which is why some results prompt maternal investigation.

The chance that your specific result is genuine depends on the condition and your prior risk. For trisomy 21 in an older mother it is relatively high. For trisomy 13, sex chromosome differences and microdeletions it can be low enough that the result is more likely wrong than right.

Many laboratories now report a personalised positive predictive value. If yours does not, ask the clinician to estimate it against your age and any other screening results. That single number changes how the next two weeks feel, and it is a reasonable thing to insist on.

What confirmatory testing involves

Chorionic villus sampling Usually 10–13 weeks. Samples placental tissue via the abdomen or cervix under ultrasound guidance. Faster access to a result at earlier gestation, though it can itself detect placental mosaicism, occasionally requiring a follow-up amniocentesis.
Amniocentesis Usually from 15 weeks. Samples amniotic fluid containing fetal cells under ultrasound guidance. Directly reflects the fetus.
Analysis Rapid testing (FISH or QF-PCR) gives results for the common trisomies within a few days; full karyotype or chromosomal microarray takes longer and detects considerably more.
Risk Roughly 0.1–0.2% procedure-related loss above background in modern series.

Working through what comes next

The waiting is the hard part, and it is worth using rather than enduring.

  1. Arrange genetic counselling. A counsellor or specialist can explain what the specific condition involves, the range of outcomes, and what confirmation will and will not tell you. Book a specialist teleconsultation.
  2. Get accurate information about the condition itself, ideally including from families living with it, rather than from the most alarming material online. Outcomes for several of these conditions, particularly the sex chromosome differences, are far more variable and often milder than initial impressions suggest.
  3. Do not make irreversible decisions before confirmation. This is the single most important point on the page.
