A number outside the printed range is not automatically a problem, and a number inside it is not automatically reassurance. Here is how to tell the difference.

What a reference range actually is

Laboratories establish a range by measuring a reference population and reporting where the central 95% falls. Two consequences follow immediately.

First, about 1 in 20 healthy people will sit outside the range for any given test. Order twenty markers on a healthy person and, statistically, you should expect roughly one flagged result. This is the main reason large untargeted panels generate anxiety rather than insight.

Second, the range describes a population, not a clinical decision threshold. Some markers do have decision thresholds — HbA1c for diabetes, for example — but many hormone ranges do not. Being slightly outside a range is a prompt for context, not for treatment.

Units, and the false alarms they cause

The same result can look wildly different in different units. Two examples cause most of the confusion:

AMH Reported in ng/mL or pmol/L. The numerical values differ by roughly sevenfold, so a value that looks alarming in one unit may be unremarkable in the other.
Testosterone Reported in nmol/L or ng/dL. Comparing a nmol/L result against a ng/dL cut-off found online is a frequent source of unnecessary alarm.
Vitamin D ng/mL or nmol/L, differing by a factor of about 2.5.

Check the unit before comparing your result to anything. When in doubt, compare only against the range printed on your own report.

Why the same person gets different numbers

Two kinds of variation sit behind every result. Analytical variation comes from the assay itself. Biological variation comes from you — time of day, cycle day, sleep, recent illness, exercise, stress, hydration and posture during the draw.

For hormones with a strong rhythm, biological variation dominates. Testosterone can vary substantially between two morning draws in the same week without anything having changed. This is exactly why guidelines require two morning measurements before diagnosing testosterone deficiency.

The practical implication: a small change between two results is usually noise. A meaningful trend needs several results taken under consistent conditions, ideally at the same laboratory. Have the trend reviewed with a MedEx doctor rather than reacting to individual points.

When an abnormal result does need prompt attention

Most borderline results warrant a repeat and a conversation. Some do not wait:

  • Markedly raised testosterone or DHEA-S in a woman with rapidly progressing virilisation — deepening voice, rapid hair growth, clitoromegaly.
  • Markedly raised prolactin, particularly with headache or visual field disturbance.
  • A clearly low morning cortisol with symptoms of adrenal insufficiency — weight loss, dizziness on standing, nausea, salt craving, skin darkening.
  • Clear hyperthyroidism with palpitations, marked weight loss or atrial fibrillation.
  • An IGF-1 well above the age-adjusted range with features of acromegaly.

Five questions to ask about any hormone result

  1. Was the sample taken at the right time? Wrong hour or wrong cycle day makes the number uninterpretable, regardless of what it says.
  2. What was I taking? Hormonal contraception, HRT, steroids, biotin and DHEA supplements all shift results.
  3. Was I well? Acute illness distorts thyroid, testosterone and IGF-1 results.
  4. Are the context markers there? SHBG with testosterone; estradiol with day-3 FSH; TSH almost always.
  5. Would a different result change what I do? If not, the test probably did not need ordering, and a borderline value does not need chasing.
